AltaMed Health Services is a non-profit health care provider that has been a safety net for underserved communities in Los Angeles and Orange Counties for more than 40 years. As one of the largest independent Federally Qualified Community Health Centers (FQHC) in the U.S., AltaMed delivers more than 930,000 annual patient visits through its 46 sites in Los Angeles and Orange Counties. AltaMed serves the entire family with primary medical care, dental clinics and complete senior long-term care services, including senior case management, and the Program of All Inclusive Care for the Elderly (PACE). AltaMed also delivers disease management programs, heath education, youth services and specialty HIV/AIDS care.
AltaMed Health Services provides services in a culturally sensitive manner that has earned it a long-standing reputation in the community as a trusted provider for Latino and multi-ethnic populations. Using a strategic, multi-tier, multi-touch outreach campaign, AltaMed has been able to reach out and educate Los Angeles and Orange County residents in need of care.
As a recent example, AltaMed initiated a strong grassroots outreach and education campaign for Covered California that resulted in more than 18,000 people educated about and enrolled for health coverage from October to the first week in April.
